Hi Andreas,
Yes, replacing the "card reader" solved the problem. I did see the error message once just recently, so I am keeping an eye on it but I hope it was just a rogue message.

Oh joy . My ACC/PAS error message has returned. It only occurs on engine start. The OPC has had a look at it and says everything is working normally. It did it again as I was leaving the garage and when I told them, they asked "is there any rain on the sensor?".
My view is that the message happens when the car is checking its systems at start up and somehow thinks the ACC/PAS system is not working properly. The message quickly disappears (after 5-10 sec?) and the ACC works completely normally. I think the ACC has to be switched on for this to happen so it may require driver memory to be present so that it remembers that the ACC was switched on for the last journey.
I'm going to monitor it over the next month whilst cleaning the sensor before every engine start and videoing the dash so that I can prove the problem to the OPC.
Anyone else getting this now?

I used to get this occasionally on my SD I put it down to being parked close to garage wall or the ball I dangle from the garage beam that I use to stop.
My other theory was it was low voltage, ie if I just flicked the key without waiting for the glow plug light to go out the car would pause for a while as the glow plugs were activated and then start automatically. Whereas doing glow plugs manually it was always ok.

Yup, I am getting it occasionally, always on start, clears within a second and leaves no message.
Dave, that definitely rings true. I notice I get the error message when there is a delay in the engine start but I hadn't realised that it might be related to glowplugs. I do tend to stick the key in and just turn it quickly. I have also heard the garage talk about a "low-voltage issue".
