What are the signs of failure?

Mine were a rumble/vibration on moving off on a medium throttle, particularly going up hill, but would not do it on a large throttle opening. Best described as like running over a rumble strip on side of road. In reverse perversely it was less noticeable. Took it in to OPC who said straight away it was faulty. Since it has been replaced I am amazed at how much smoother the car is. To be honest difficult to believe I never noticed before. Car had done about 25K when it was changed under the extended warranty.
